Tesamorelin is a synthetic analogue of endogenous Growth Hormone Releasing Hormone (GHRH), comprising the full 44-amino-acid GHRH(1–44) sequence conjugated to a trans-3-hexenoic acid moiety at the N-terminus. This modification substantially improves metabolic stability by protecting the N-terminal Tyr–Ala bond from dipeptidyl peptidase IV (DPP-IV) cleavage — the primary degradation pathway of native GHRH — while preserving full agonist activity at the GHRH receptor (GHRHR) on anterior pituitary somatotroph cells.
Upon receptor binding, Tesamorelin activates the Gαs–adenylyl cyclase–cAMP–PKA signalling cascade, stimulating the synthesis and pulsatile release of endogenous Growth Hormone (GH). Unlike exogenous GH administration, this mechanism preserves the physiological feedback regulation of the GH axis, including somatostatin-mediated inhibition. Downstream, GH stimulates hepatic IGF-1 production and activates GH receptors in peripheral tissues.

Reconstitution Guidance
Add Bacteriostatic Water slowly down the side of the vial — do not inject directly onto the lyophilised cake. Swirl gently; do not vortex or shake. Allow to stand 2–3 minutes until fully dissolved. Solution should be clear and colourless. Store refrigerated and use within 14–21 days.
